Already connected, or just beginning?

We have a care manager

A SIMPLE FIRST CONTACT

Your family’s questions.

Review your own contact details before sending. A confirmation and reference appear only after the request is saved.

What should I put in the form?
Use your own name, callback number, and contact preferences. You may choose “Children’s support / OPWDD” as a general care interest. Do not enter your child’s name, diagnoses, insurance identifiers, or medical documents.
Does an OPWDD approval mean care can start?
No. Program eligibility and a confirmed Heritage care arrangement are separate. The requested service, provider participation, assessments, payment, authorization, and staffing must be confirmed individually.
What should I prepare for the intake conversation?
Have your borough, preferred language, general daily support needs, requested schedule, and whether a care manager is involved in mind. Ask intake how required documents should be shared.
Can a care manager use this family form?
Professionals should request a professional callback with their own work details and organization, or review the referral process. Call before sending patient information or clinical records.
