How do I get started?
Call 718-750-3550 or request a callback. Our intake team will discuss your needs, location, payment options, and the next steps for an assessment.
Does Heritage serve my neighborhood?
Heritage serves all five New York City boroughs: Brooklyn, Queens, Manhattan, the Bronx, and Staten Island. Tell us your ZIP code, preferred language, and schedule so the team can confirm availability.
Can you help me understand Medicaid home care?
Our team can explain the intake process and help you navigate Medicaid-related questions. Eligibility, covered services, and authorized hours are determined through the applicable program and health plan; they are not guaranteed by a website screening. Explore Medicaid & MLTC next steps.
Can I request a caregiver who speaks my language?
Yes. Let us know your preferred language, care schedule, and location. We consider these preferences when discussing caregiver availability. A specific match must be confirmed by our team.
Can home health aides (HHAs) or personal care aides (PCAs) apply?
Yes. Visit our caregiver page to send a contact request. Our recruitment team will discuss credentials, availability, onboarding requirements, and current opportunities. Applying does not guarantee a position or specific hours.
What do home health aides (HHAs) and personal care aides (PCAs) help with?
Discuss personal care, meals, mobility, household routines, medication reminders, and companionship. Specific tasks depend on the assessed plan, authorization, and the caregiver’s role.
Can you help if my authorized hours no longer meet my needs?
Ask your plan or program about reassessment and current documentation requirements. Heritage can help coordinate the care conversation; the plan or program decides any change in authorized services.
Can I move from another agency to Heritage?
Call to discuss the applicable plan, service authorization, requested schedule, and timing. A transition must be coordinated and the new service start confirmed.
Where do I begin with NHTD or TBI support?
If you have a service coordinator, ask them to contact intake. If you are still exploring, call with general questions about the pathway and the appropriate program contacts. Service coordinators: Request a professional intake callback.
Does Heritage provide every OPWDD service?
No. Discuss the requested home care role with intake and your care manager. Program eligibility, provider participation, and authorization must be verified for the specific service.
Can you coordinate therapy with home care?
Call about the home care request and existing therapy providers. PT, OT, and speech treatment require appropriately qualified clinicians and their own service arrangements.
What should a discharge planner confirm?
Confirm the requested service, responsible providers, assessment and authorization requirements, staffing, and the actual start arrangement. A proposed discharge date alone does not confirm care. Use the professional referral checklist.
Can I arrange overnight or live-in care?
Tell intake the requested days and hours. Extended schedules, overnight arrangements, or live-in requests depend on assessment, appropriate service terms, and confirmed availability.
What is Heritage Academy?
It is the caregiver orientation and ongoing education pathway described on this site. Ask recruitment about current topics and schedules. It is not represented as an initial HHA/PCA certification course. Already with Heritage? Help with assigned training.
How do the website request forms work?
The callback and caregiver forms let you review your details, then send a contact request to Heritage’s private intake inbox. The care finder’s separate email option prepares a draft in your email app. Call to confirm the appropriate channel for clinical referrals.
