COMPLIANCE & ETHICAL CONDUCT

Integrity in care.
A clear way to speak up.

Heritage’s public standards, non-retaliation commitment, and ways to report a compliance concern.

REPORTING A CONCERN

Speak up about fraud, waste, or abuse.

Report suspected false visit records, improper billing, referral payments, misuse of funds, or other compliance concerns. A concern can be raised internally or directly with the appropriate state agency.

  1. Contact Heritage’s Compliance Officer.

Call 718-750-3550 and ask to speak with the Compliance Officer. For a written report without your name, use the route below.

  2. Use the written reporting route.

Send a sealed letter marked “Confidential — Attention: Compliance Officer” to the address below. You may omit your name and contact details. If you do provide contact details, the agency can follow up with you.

  3. Describe the facts and preserve records.

Explain what happened, relevant dates, and the nature of the concern. Preserve original records. Ask the Compliance Officer for the appropriate secure way to provide supporting material; do not send patient records through the website’s callback form.

  4. You may report directly to OMIG.

Call the New York State Office of the Medicaid Inspector General at 1-877-873-7283 or use its official allegation process. OMIG permits reports without identifying yourself. Internal permission is not required.

Confidentiality is protected to the extent permitted by law. Required disclosures or an investigation may limit confidentiality. For an immediate emergency, call 911.

NON-INTIMIDATION & NON-RETALIATION

Good-faith reporting must be protected.

Heritage prohibits intimidation, retaliation, or discrimination against employees, contractors, or agents who raise compliance concerns in good faith. This protection applies regardless of the investigation’s outcome. Report suspected retaliation to the Compliance Officer or the appropriate government agency.

New York Labor Law §§ 740 and 741 provide additional protections for covered workers and activities under their respective terms. Confidentiality obligations do not prevent lawful reporting to government agencies.
