PERSONAL SUPPORT. CLEAR NEXT STEPS.

Support for daily life at home.

NHTD is a New York Medicaid waiver pathway for eligible people who need support to live in the community. Heritage can discuss home care and Home and Community Support Services (HCSS) needs, and coordinate next steps with your service coordinator. A waiver service plan and program approval are separate from an agency’s general home care license.

01

Daily living support

Discuss assistance with personal care, meals, household routines, and other tasks included in the authorized plan.

02

Cueing and supervision

Explain the reminders, prompts, and oversight needed during daily activities. HCSS may combine practical assistance with supervision described in the service plan.

03

A connected plan

Bring your service coordinator into the conversation so the requested tasks, schedule, documents, and responsible providers are clear.

04

An individual start

Program participation, the requested service, authorization, and caregiver availability must be confirmed before a start date is arranged.

Already have a coordinator?

Ask your coordinator to contact Heritage about the approved plan, requested HCSS, service area, and appropriate referral channel.

Exploring NHTD for the first time?

Ask intake where to begin and who handles program screening and enrollment. The program’s reviewers determine eligibility and approvals; a website inquiry does not enroll you.

QUESTIONS ABOUT THIS PATHWAY

Start with clarity.

Is NHTD the same as regular Medicaid home care?
It is a separate waiver pathway with its own eligibility, planning, and approval process. Ask which route applies to your situation.
Does HCSS mean every medical service is included?
No. The approved plan defines HCSS tasks. Nursing, therapy, equipment, and other services may involve separate orders, providers, or approvals.
