MEDICAID, MANAGED CARE & PRIVATE PAY

Paying for home care.
A clearer next step.

Medicaid, managed long-term care (MLTC), or private pay: start with your situation, then discuss the service and payment arrangement with intake.

WHERE ARE YOU STARTING?

Choose your first conversation.

You don’t need to know every program term. Start with the situation that fits your family.

I need to apply

Ask where to begin with a Medicaid application and which supporting documents may be needed.

What to prepare
  • Confirm the appropriate application route.
  • Ask who handles the eligibility decision.
  • Clarify how to follow up on the next step.

I already have Medicaid or a plan

Have your exact plan name ready for a phone conversation about services and Heritage’s current participation.

What to prepare
  • Confirm the specific plan and requested service.
  • Ask about assessments and authorization requirements.
  • Discuss location, language, and the requested schedule.
Discuss my care arrangement

My care needs or hours have changed

Contact your plan or responsible program about changed needs, reassessment, and its documentation requirements.

What to prepare
  • Ask whom to contact about reassessment.
  • Clarify what current documentation is needed.
  • Confirm any new authorization before arranging a changed schedule.
Prepare my care conversation

I want to change agencies

Discuss your current plan, authorized service, requested schedule, and proposed transition timing.

What to prepare
  • Coordinate with the plan and current agency.
  • Confirm Heritage’s service acceptance and staffing.
  • Verify the new start arrangement before a change.
Talk with Heritage intake

Eligibility, provider participation, covered services, authorized hours, and staffing require individual confirmation. Additional hours or a proposed start date are not automatic approvals.

PAYING DIRECTLY

Understand the charges before care begins.

Private pay is a payment arrangement for the care you are considering. Discuss your requested support, schedule, assessment needs, and current service terms with Heritage.

KNOW WHO HANDLES EACH STEP

A connected conversation.

Depending on the pathway, assessments and planning may involve the New York Independent Assessor, your health plan, physician, or program coordinator.

Your plan or program

Confirm the applicable assessment, eligibility, service authorization, and provider arrangements with the responsible team.

Heritage intake

Discuss service fit, current participation, your borough, language, requested schedule, and confirmed staffing.

Benefits questions

Bring questions about income, resources, or a pooled income trust to an appropriately qualified benefits or legal adviser.

A FEW PRACTICAL DETAILS

Make your first call useful.

Have your borough, preferred language, general support needs, and requested schedule in mind. Tell intake whether you already have a plan or service coordinator.

CLARITY BEFORE YOU DECIDE

Your payment questions.

Talk with intake and the applicable plan or program about your individual arrangement.

Does a conversation with Heritage approve coverage or hours?
Your applicable program or health plan determines eligibility, covered services, and authorized hours. Heritage can discuss the requested home care arrangement, current participation, and staffing. A website inquiry does not approve services.
What should I ask if I already have a health plan?
Confirm the exact plan name, Heritage’s current participation, the requested service, assessment needs, and authorization requirements. Discuss your borough, language, and requested schedule with intake.
Where can I find private-pay costs?
Call 718-750-3550 for the current hourly or daily charges. Ask for written details of applicable minimums, assessment charges, fees, billing, and cancellation terms. Read service and charge information.
Can I move from another home care agency?
Call to discuss the current plan, authorized service, requested schedule, and transition timing. Coordinate with your plan and current agency, and confirm the new service start arrangement.
What information belongs in a website request?
Use your own contact details and general care preferences. Keep diagnoses, medical records, insurance numbers, Social Security numbers, and financial documents out of the form. Intake will confirm the appropriate channel for any required information.
