START WITH EVERYDAY LIFE

Care built around your child’s day.

Tell us what a familiar day looks like and where help is needed. We’ll discuss the requested support with your family and the team already involved.

Personal routines

Discuss dressing, hygiene, meals, mobility, and other daily tasks within an assessed and authorized home care plan.

Your family’s preferences

Share your child’s communication style, familiar routines, preferred language, and the role family members want in daily care.

A connected care team

Help us understand existing nursing, therapy, school, and care management supports so each provider’s home care role is clear.

YOUR OPWDD STARTING POINT

Already connected, or just beginning?

OPWDD is New York’s Office for People With Developmental Disabilities. Start with the situation that fits your family.

ALREADY HAVE A CARE MANAGER

Bring your existing team into the conversation.

Ask your care manager to discuss the requested home care role with Heritage and how it fits alongside your child’s existing Life Plan.

  • Clarify the requested service, borough, and schedule.
  • Review the authorization, payment, and provider arrangement.
  • Confirm the appropriate referral channel before sharing records.

NEW TO OPWDD

Begin with official eligibility guidance.

OPWDD’s Front Door is the starting point for program information. Heritage can discuss your home care question alongside that process.

  • Explore OPWDD’s getting-started information.
  • Ask about the eligibility process and care management options.
  • Speak with Heritage about the home support you are exploring.

Heritage’s role is home care and coordination alongside your existing team. Your care manager remains responsible for care management; OPWDD determines program eligibility and authorizes OPWDD services. The specific service, provider participation, assessments, payment, authorization, and staffing must be confirmed individually.

A SIMPLE FIRST CONVERSATION

You don’t need every answer to begin.

Start with general support needs and your own contact details. Our intake team can explain the next steps and what needs to be confirmed.

YOUR QUESTIONS MATTER

A clearer next step for your family.

Call 718-750-3550 to discuss your situation. Specific care arrangements require individual review.

How do we get started with Heritage?
Call 718-750-3550 or request a callback with your own contact details. Discuss your borough, preferred language, general support needs, and whether a care manager is already involved. Ask the team for the appropriate channel before sharing your child’s records.
Does OPWDD eligibility confirm services with Heritage?
OPWDD eligibility and a service request are separate from a confirmed home care arrangement. The specific service, provider participation, authorization, payment pathway, and staffing must be reviewed individually.
Can you promise an aide with a specific language or background?
We welcome those preferences and use them in staffing discussions. A particular match or schedule must be confirmed by the team.
Does this include ABA or Early Intervention?
Those are distinct services. Ask intake and your care manager which qualified provider is responsible for each requested service. This page describes Heritage’s home care and coordination role.
