PERSONAL SUPPORT. CLEAR NEXT STEPS.

Build the home plan before departure.

Returning home can involve several organizations. Heritage intake can discuss the requested aide or nursing support, confirm service fit, and explain what is needed to consider a start. The discharge team and treating clinicians remain responsible for medical discharge instructions and decisions.

01

Discuss the home routine

Clarify personal care needs, mobility assistance, meals, access to the home, and who else will be available.

02

Identify each provider

Confirm who arranges aide services, nursing, therapy, equipment, prescriptions, transportation, and follow-up appointments. These may be separate arrangements.

03

Align the approvals

Discuss the health plan or private payment arrangement, required orders, assessments, authorizations, and referral documents.

04

Confirm the actual start

A proposed discharge date is not a confirmed caregiver start. Ask intake to verify accepted services, staffing, schedule, and the contact for changes.

For families

Keep a list of provider contacts and the next action each team has agreed to. Ask whom to call if the home support plan changes.

For professionals

Start with a general service-fit call. Heritage will confirm the appropriate referral channel before patient records or identifiers are shared.

QUESTIONS ABOUT THIS PATHWAY

Start with clarity.

Can care start today?
Call intake with the requested timing. Acceptance and the earliest start depend on the service, required documents and approvals, and confirmed staffing.
Does working with a certified home health agency make Heritage a CHHA?
No. Heritage is a licensed home care services agency (LHCSA). A CHHA or another provider may coordinate separate services or request aide support through an appropriate arrangement.
