Talk with Heritage about coordinating home care with physical, occupational, or speech therapy delivered by qualified providers.
Discuss this care pathwayPERSONAL SUPPORT. CLEAR NEXT STEPS.
A home care aide, a nurse, and a therapist bring different skills to the home. Heritage helps clarify the requested home care role and coordinate with the clinicians and agencies involved. Therapy may require a prescribing clinician, assessment, a qualified therapy provider, coverage review, and its own service arrangement.
Ask the therapy provider about mobility, balance, strength, and a safe activity plan. Tell intake how personal care support needs to fit around therapy visits.
Discuss daily activities, home routines, adaptive approaches, or equipment recommendations with the occupational therapist.
Questions involving communication or swallowing should go to the qualified speech-language provider. Share relevant instructions through the appropriate care channel.
Caregivers follow the assigned home care plan and tasks within their scope. They do not replace a therapist or independently change prescribed exercises or clinical instructions.
Ask who can share the relevant home care instructions and how changes will reach the nurse or care coordinator.
Call intake to discuss referral or coordination options. Confirm the treating provider, orders, payment, and appointment availability separately.
